ADA Paratransit is a shared ride, advanced reservation, origin-to-destination service for persons with disabilities who are unable to use the public bus service because of their disability.

Please note that any information given on this application will be kept confidential and shared only with professionals involved in providing the paratransit service on an as needed basis.

THIS APPLICATION WILL BE ACCEPTED AT ANY ADA PARATRANSIT PROVIDER IN THE STATE OF CONNECTICUT.

Passengers must be approved through an eligibility process managed by your local Transit District. Eligible applicants must be at least 60 years young or have a disability. Once approved, M7 will create your personal online account to manage expenses and view your travel history.

The Kennedy Collective Travel Training is a nationally-acclaimed program teaching people with cognitive, sensory, and physical disabilities how to independently and effectively use the local public bus and public rail systems throughout Connecticut.

There are follow-up services including: ongoing communication between the individual and instructor, ensuring safety to and from destinations, and providing re-training where necessary.

Freedom Ride is a prepaid reduced fare voucher program that offers a 50% discount from the normal taxicab rate, with a minimum purchase of $25 initially. It is available to ADA eligible passengers who need transportation beyond the traditional ADA paratransit service area.

MTM Health manages non-emergency medical transportation (NEMT) services in Connecticut.

MTM CT Medical Necessity Form

This form is to be completed by a licensed health care provider. It is the member’s responsibility to make sure this form is received by Veyo. The form will not be processed for the requested authorizations if it is missing medical necessity information or justification. Please ensure that the form is completed accurately and appropriately for approval.

MTM CT Minor to Consent Travel Form

This authorization form must be completed by a parent or legal guardian before a child between the ages of 12-15 can travel without an adult escort or aide.

MTM Link gives members tools for managing non-emergency medical transportation (NEMT) rides without having to call MTM.

Veyo CT Medical Necessity Form

Veyo CT Minor Consent to Travel Form

This authorization form must be completed by a parent or legal guardian before a child between the ages of 12-15 can travel without an adult escort or aide.

Veyo CT MRB Driver Registration Form

Please remember to include a photocopy of the driver’s license and vehicle insurance when submitting this form. Forms submitted without these attachments will not be approved.

Veyo CT Mileage Reimbursement Form

This form can be used to request reimbursement for driving a HUSKY Health member to a healthcare appointment. This form can be used for up to 5 medical appointments of mileage reimbursement from the member’s home address to a single medical facility location. Veyo must receive the completed form via mail, email, or fax within 30 days of the first medical appointment listed on the form.
